I give Eden Veterinary and its authorized representatives full authority to perform humane euthanasia, and I authorize pre-euthanasia sedation and/or anesthesia as clinically indicated for my pet's comfort. I understand that once the euthanasia procedure has begun, it is irreversible.

I have read and agree to Eden Veterinary's Terms of Service which include sedation and anesthesia details, normal events that may occur during euthanasia, aftercare and remains-handling responsibilities, the cancellation policy fees, after-hours surcharges, and electronic-records consent.
